Care Manager, Telephonic Behavioral Health 2

Location: Remote in South Carolina; must reside in South Carolina. Field work is mandatory, with travel up to 50% of the time.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ behavioral, physical, environmental, and psychosocial health needs in a telephonic care-management environment.
• Guide members and families toward appropriate healthcare, wellbeing, and community resources.
• Develop care plans when needed and monitor progress toward desired outcomes through ongoing assessments and evaluations.
• Use behavioral-change, health-promotion, coaching, and wellness strategies to support members.
• Identify and resolve barriers that prevent effective care and coordinate with internal partners and members.
• Travel for field work and occasionally attend hirify.global office training or meetings.

Requirements

• Active, unrestricted SC Licensed Masters Social Worker (LMSW), Licensed Professional Counselor (LPC), or compact Registered Nurse (RN) license required.
• Must reside in South Carolina and be willing to travel.
• At least 2 years of experience working as a behavioral health professional.
• Experience with behavioral change, health promotion, coaching, and wellness.
• Proficiency with electronic information systems and Microsoft Office tools, including Word, Outlook, and Excel.
• A private home office with a locked door is required to protect member PHI and HIPAA information.

Nice to have

• Experience supporting patients telephonically.
• Experience in Community Mental Health, crisis response, Assertive Community Treatment, or substance use disorder treatment.
• Experience working with both children and adults.

• Full-time schedule of 40 hours per week, Monday through Friday, 8:00 AM to 5:00 PM.
