Health Home Care Manager (Healthcare)

Location: Jamaica, New York; field-based across Brooklyn and Queens with frequent travel to patients' homes

Salary: $24–$26 USD per hour

What you will do

• Outreach to potentially eligible patients, obtain consents, and enroll them in the Health Home Care Management Program.
• Conduct functional eligibility screenings, comprehensive assessments, and reassessments covering medical, mental health, substance use, financial, housing, and support needs.
• Develop, implement, and coordinate patient-centered care plans for patients with chronic conditions.
• Provide direct service, facilitate referrals, address access barriers, connect patients with community resources, and coordinate appointments and care.
• Collaborate with medical providers, pharmacies, insurers, hospitals, family caregivers, and other care providers.
• Conduct home visits, provide crisis intervention, document services, and maintain patient confidentiality.

Requirements

• Associate's degree required.
• At least 2 years of care management experience.
• Ability to travel in the field across at least Brooklyn and Queens, including home visits and in-person patient appointments.
• Knowledge of local community resources and experience with data reporting, documentation, and outcomes.
• Strong communication, assessment, empathy, and crisis-management skills.
• Ability to manage high-volume caseloads and work with data management systems such as Salesforce.
