Field Care Coordinator - Bilingual Preferred

Location: Tampa, Florida; must reside in Hillsborough County, Florida. The role involves 75–90% local travel to members' homes, assisted living facilities, long-term care facilities, and other care settings.

Starting salary: $60,000–$65,000 per year based on qualifications and experience. Posted pay range: $53,700–$72,600 per year.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess member needs and establish individualized care plans.
• Plan and implement interventions that support members' health and well-being.
• Coordinate services with providers, payer sources, interdisciplinary teams, families, and community resources.
• Monitor case management plans against members' personal goals and evaluate outcomes.
• Guide members and families toward appropriate care and social services.

Requirements

• Bachelor's degree in a health and human services field.
• At least 2 years of related experience.
• Must reside in Hillsborough County, Florida.
• Valid state driver's license, personal vehicle liability insurance, and willingness to travel locally 75–90%.
• A separate room with a locked door is required for a private home office.
• Patient-facing work requires tuberculosis screening and Florida Level 2 background screening.

Nice to have

• Experience with electronic case note documentation and multiple computer systems.
• Experience working with Medicare and Medicaid recipients.
• Experience in health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and community resources.
• Bilingual communication skills.
