LTSS Service Care Coordinator

Location: Remote within Texas; must reside in the state of Texas. The role may include on-site visits to assess member needs.

Salary: $22.94–$38.79 per hour

What you will do

• Assess member needs, available resources, and appropriate care outcomes.
• Develop and monitor long-term care plans and coordinate services with providers, specialists, families, and caregivers.
• Connect members with community resources related to employment, housing, independent living, justice, foster care, and participant direction.
• Educate members and caregivers about healthcare benefits, service options, referrals, procedures, and provider instructions.
• Document member information and care management activities in accordance with state, federal, and payer requirements.
• Identify opportunities to improve quality of care and cost-effective service delivery.

Requirements

• Bachelor's degree and at least one year of related experience, or equivalent applicable experience.
• Must reside in Texas.
• Direct experience within three of the last five years working with people with disabilities or vulnerable populations with chronic or complex conditions, including children and young adults.
• Ability to coordinate with long-term care providers, members, families, caregivers, and community resources.
