Health Coach - Shelbyville, IN - 24 hours per week

Location: Onsite in Shelbyville, Indiana, United States. The role includes virtual and in-person presentations and participation in client engagement events.

Salary: $39,000–$51,000 per year for a 0.6 FTE position.

What you will do

• Conduct individual and group coaching sessions focused on behavior change, wellness, and chronic condition management.
• Develop personalized wellness plans, set goals with patients, and provide resources to track progress.
• Collaborate with healthcare providers on chronic condition coaching based on national clinical guidelines.
• Assist patients with medical appointments and internal referrals.
• Facilitate group challenges, educational workshops, biometric screenings, and other health promotion programs.
• Coordinate wellness initiatives and available resources with clients, care teams, carriers, EAP providers, and other vendors.

Requirements

• Bachelor’s degree in health science, counseling, psychology, nutrition, social work, or a related field.
• NBHWC certification required.
• At least two years of experience as a health coach, including individual and group coaching.
• Strong background in health promotion, chronic condition management, and behavior change methodologies.
• Strong communication, organization, critical thinking, problem-solving, customer service, and time management skills.
• Technical proficiency with electronic medical record software and Microsoft 365, including Teams, Word, Outlook, Excel, and PowerPoint.

Nice to have

• Master’s degree in a relevant field.
• Experience presenting to small and large groups in virtual and in-person settings.
