Care Coordinator II (Behavioral Health)

Location: Field-based role in Fayetteville, Arkansas; candidates must reside in Washington, Benton, or Madison County, Arkansas, or a border city. Requires approximately 30% travel and home visits.

Salary: $17.84–$28.02 per hour

What you will do

• Conduct member outreach by phone and through home visits to discuss care plans, resources, questions, and next steps.
• Coordinate care activities with care managers, healthcare providers, community organizations, members, and caregivers.
• Support member and provider inquiries related to care plans and services.
• Assist with service assessments and screenings, identify member needs, and make appropriate referrals.
• Document and maintain member records in accordance with regulatory and contractual requirements.
• Provide education about available benefits and resources while supporting continuity of care.

Requirements

• High school diploma or GED and 1–2 years of related experience.
• For the Arkansas Total Care plan: a bachelor's degree in social science or a health-related field, or a high school diploma with at least one year of experience coordinating care for clients with developmental or intellectual disabilities or behavioral health needs.
• Must reside in Arkansas or a border city, with the assigned territory covering Washington, Benton, or Madison County.
• Valid driver's license required.
• Child and adult maltreatment checks and drug screening are required before hire and on a recurring basis.
• Approximately 30% travel is required.
