Behavioral Health Intake Assessor/Case Therapist II (Behavioral Health)

Location: DeKalb, Illinois; full-time onsite position. The schedule is 40 hours per week, Monday through Friday, primarily during the day, with two evening shifts per week until 8:30 PM.

Salary: $33.76–$47.26 per hour.

What you will do

• Complete integrated behavioral health assessments and treatment plans in accordance with Medicaid and Rule 2060 requirements.
• Evaluate client needs, strengths, goals, and resources, and make clinical recommendations for mental health and addiction-related needs.
• Review cases with an interdisciplinary treatment team and develop individualized plans.
• Consult with center staff and external service agencies on assessment, treatment planning, and case-specific issues.
• Refer and connect clients with human services programs and serve as a liaison to other agencies.
• Prepare timely clinical documentation and train providers and non-providers in relevant service specialties.

Requirements

• Master’s degree in behavioral health.
• Two years of supervised clinical experience in a behavioral health setting.
• Independent clinical practice license as an LCPC, LCSW, LMFT, APN, or clinical psychologist.
• Valid Illinois driver’s license, current auto liability insurance, and an annual driving abstract required at hire and annually.
• Ability to work a 40-hour Monday–Friday schedule, including two evening shifts per week until 8:30 PM.
