CHOICES Care Coordinator (Healthcare)

Location: Must live in Shelby County, Tennessee, and travel to members' homes for face-to-face visits. Schedule: 8:00am–5:00pm EST, with no on-call requirement.

What you will do

• Partner with members and families to identify supports and services that advance health, employment, independence, and community living goals.
• Collaborate with clinical and social support professionals to address physical, behavioral health, and long-term service needs.
• Conduct objective face-to-face visits and assess physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Use authorization criteria to arrange appropriate home- and community-based services.
• Confirm that authorized services are delivered and members' needs are being met.

Requirements

• At least 2 years of clinical experience; 3 years of clinical-setting experience is preferred in the role description.
• Active Tennessee registered nurse license, or an active license in a Nurse Licensure Compact state; alternatively, a Master's in Social Work with an unrestricted LCSW, LMSW, or LAPSW license.
• Residence in Shelby County, Tennessee is required.
• Valid driver's license, with auto insurance compliance and an acceptable motor vehicle record for routine business driving.
• Strong customer service, communication, interpersonal, organizational, critical-thinking, time-management, and prioritization skills.
• Basic PC skills, including Microsoft Office and email; a digital literacy assessment is required.
