Palliative Care Nurse (Palliative Care)

Location: Remote within the USA; shift flexibility is required to cover members in Pacific Time.

Salary: $80,000–$100,000 per year

What you will do

• Provide virtual, home-based palliative care by phone and video using compassionate, evidence-based clinical practice.
• Coordinate care and manage cases, including DME orders, home health collaboration, and communication with primary care providers.
• Maintain a patient panel, conduct risk-based check-ins, monitor active issues, and make referrals to palliative social workers and nurse practitioners.
• Participate in daily team huddles, weekly interdisciplinary palliative rounds, and incoming patient-call triage.
• Collaborate with Devoted Medical teams and help develop palliative education initiatives and the multi-state care program.

Requirements

• Unrestricted RN license; compact licensure is preferred, with the ability to obtain additional state licenses within 90 days if requested.
• At least 5 years of RN experience, including at least 2 years in hospice or palliative care.
• CHPN credential, or willingness to obtain it within the first six months of employment.
• Active Basic Life Support certification.
• Willingness to work flexibly across time zones and cover Pacific Time members as needed.
• Ability to work independently in a technology-enabled, work-from-home environment with strong communication and clinical judgment.

Nice to have

• ELNEC or EPEC training.
• Bilingual ability in English and Creole, Spanish, or Vietnamese.
• ICU, oncology, geriatrics, inpatient nursing, or program development experience.
