Cardiology Patient Navigator

Location: Remote, Lima, Peru. Must work consistently during US business hours in Central Time (CT).

Salary: USD 5–7 per hour.

What you will do

• Manage patient scheduling, reminders, follow-up workflows, and care coordination in EMR/EHR systems.
• Conduct telephonic and remote outreach to reinforce care instructions and coordinate next steps.
• Support cardiology care pathways, including CHF management protocols and remote patient monitoring.
• Track testing schedules, appointments, and continuity of care for a caseload of 150+ patients.
• Escalate clinical concerns to the appropriate provider within scope without diagnosing or prescribing.
• Maintain clinical documentation and support billing documentation and insurance coordination.

Requirements

• Clinical healthcare background as a CNA, MA, RN, or equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience with care coordination or care management, preferably in telephonic or remote settings.
• Experience using EMR/EHR systems and strong patient-centered communication skills.
• B2+ English proficiency, assessed during screening.
• Availability during US Central Time business hours.

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, home health, outpatient cardiology, or telemedicine.
• Exposure to remote patient monitoring, CCM, utilization review, medical billing, or prior authorization.
• Previous experience working as a contractor or in startup and evolving environments.
