What you will do

• Conduct telephonic clinical assessments, pain assessments, and medication reconciliations for high-acuity members.
• Coordinate care transitions and discharge plans with hospital case managers and providers.
• Educate members and caregivers about cancer diagnoses and treatment paths over the phone.
• Collaborate with payers, providers, and non-clinical team members to address social determinants of health.
• Monitor member progress and establish targeted support plans during case conferences.
• Handle urgent clinical escalations and provide clinical consult support.

Requirements

• Must be located within the lower 48 United States.
• Active, unrestricted compact Registered Nurse (RN) license.
• Degree in Nursing (ADN or BSN required; BSN preferred).
• Minimum 5 years of nursing experience, including 3 years of high-acuity adult oncology experience.
• Certification as OCN, AOCN, AOCNS, or CCM (OCN required at hire or within 2 years).
• Experience working remotely and proficiency with technology (Google Suite, Slack, EHR).

Nice to have

• Fluency in Spanish, both written and verbal.
