Complex Oncology Nurse Navigator (Medical)

Location: Remote or hybrid in the Nashville office; must be located within the lower 48 United States due to contractual limitations with accessing PHIs. Schedule: 11:30 AM–8:00 PM EST.

Salary: $46.35/hour.

What you will do

• Conduct telephonic clinical assessments, including pain assessments and medication reconciliation, while monitoring changes in member health.
• Coordinate care transitions, discharge plans, follow-up care, referrals, medications, appointments, and durable medical equipment or supplies.
• Provide education and support to oncology members and caregivers across the cancer-care continuum.
• Manage urgent clinical escalations, virtual home-safety evaluations, and clinical consultations for higher-acuity members.
• Collaborate with providers, payers, hospital case managers, and care-team colleagues to prevent readmissions and address social needs.
• Share member and provider feedback with clinical leadership and support administrative and service-improvement projects.

Requirements

• Active, unrestricted compact Registered Nurse license required; willingness to obtain additional state licenses as needed.
• ADN or BSN in Nursing required; BSN preferred.
• At least 5 years of nursing experience, including 3 years of high-acuity adult oncology experience.
• OCN, AOCN, AOCNS, or CCM certification required; OCN certification must be held at hire or obtained within 2 years.
• Experience working remotely and willingness to learn new technology, including video chat, Google Suite, Slack, and electronic health records.
• Ability to work the 11:30 AM–8:00 PM EST schedule and up to 2 holidays per year.

Nice to have

• Fluency in Spanish, written and verbal.
• BSN degree.
