Cardiology Patient Navigator (Cardiology)

Location: Remote · Global; based in Tirana, Albania; US business hours in Central Time required

Healthcare care coordination for a US-based cardiology team.

What you will do

• Manage patient scheduling, reminders, follow-up workflows, and telephonic outreach.
• Coordinate cardiology care pathways, including CHF management protocols and next steps.
• Track testing, appointments, remote patient monitoring, and continuity of care for a caseload of 150+ patients.
• Maintain timely clinical documentation and support billing documentation and insurance coordination.
• Escalate clinical concerns to the appropriate provider within scope without diagnosing or prescribing.

Requirements

• Clinical healthcare background as a CNA, MA, RN, or equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Care coordination or care management experience, preferably in telephonic or remote settings.
• Experience with EMR/EHR systems and strong patient-centered communication skills.
• B2+ English proficiency and availability during US Central Time business hours are required.
• Personal Windows or Mac laptop with at least 16 GB RAM, supported Windows 11 or macOS, Chrome, external peripherals, webcam, headset, and broadband of at least 100 Mbps download / 10 Mbps upload.

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, home health, outpatient cardiology, or telemedicine.
• Exposure to remote patient monitoring, CCM, utilization review, medical billing, or prior authorization.
• Previous contractor experience or experience in startup and evolving environments.
