LTSS Service Care Manager

Location: Remote work with approximately 50% travel to nursing facilities and other sites; candidates must reside in Pennsylvania. Preference is given to candidates in or near Lawrence, Mercer, Butler, or Beaver counties.

Salary: $27.02–$48.55 per hour

What you will do

• Evaluate member needs, available resources, and appropriate long-term care options.
• Develop and monitor personalized service care plans and coordinate required providers, specialists, and community resources.
• Coordinate communication between members, families or caregivers, healthcare providers, and partner organizations.
• Provide support and education about service options, referrals, healthcare benefits, and provider instructions.
• Conduct nursing facility, home, and other site visits when needed to assess member needs.
• Document care management activities and provide recommendations to improve quality, outcomes, and cost-effective service delivery.

Requirements

• Must reside in Pennsylvania.
• Bachelor's degree and 2–4 years of related experience, or equivalent applicable knowledge and experience.
• Experience assessing member needs and coordinating long-term care services.
• Ability to collaborate with members, caregivers, healthcare providers, and community partners.
• Ability to travel approximately 50% to nursing facilities and perform site visits.

• Previous assessment experience.
• Experience with Community Health Choices (CHC).
• Residence in or near Lawrence, Mercer, Butler, or Beaver counties.
