Remote Care Navigator (Cardiac)

Location: 100% remote within the United States; Dallas/Fort Worth area preferred. US business hours with CST/PST overlap required.

Salary: $21–$24 USD per hour, based on experience.

What you will do

• Conduct structured telephonic outreach to patients with congestive heart failure and complex cardiac conditions.
• Manage a patient caseload using risk stratification to prioritize outreach and follow-ups.
• Complete assessments covering symptoms, medications, psychosocial status, and social determinants of health barriers.
• Support Transitional Care Management follow-up within 48 hours after discharge, including medication reconciliation, red-flag screening, and appointment scheduling.
• Provide CHF self-management education and identify care gaps through population health dashboards.
• Document interventions, care plans, and patient goals according to CMS billing standards while escalating clinical concerns to supervising RN Care Managers.

Requirements

• Active Medical Assistant certification or equivalent credential such as CNA, EMT, or CHW with relevant experience.
• At least 2 years of experience in care coordination, case management, or ambulatory care.
• Familiarity with CMS PCM, CCM, and/or TCM requirements and documentation standards.
• Technological proficiency with care coordination software and EHR platforms.
• Must be based in and authorized to work in the United States; availability during US business hours with CST/PST overlap is required.
• Exceptional written and verbal English communication skills are required.

• Knowledge of cardiac conditions, especially heart failure and associated comorbidities.
• Spanish/English bilingual communication skills.
• Experience actively using AI tools to work more efficiently.
