Hospice Registered Nurse, RN, Case Manager

Location: Malden, Massachusetts and surrounding areas; field-based role with weekend, holiday, evening, and on-call coverage

Salary: $84,300.22–$107,481.20 per year based on full-time equivalent

What you will do

• Serve as case manager for the interdisciplinary hospice team and deliver nursing care to assigned patients and families.
• Assess, plan, implement, and evaluate patient care using hospice, palliative-care, and symptom-management standards.
• Coordinate care, nursing visits, triage, additional services, and communication with physicians and care-at-home staff.
• Maintain care plans, medication profiles, home health aide treatment plans, assessments, interventions, and clinical documentation.
• Provide patient and family education, advocacy, resource information, and support across different family systems.
• Manage caseloads independently, collaborate with the Clinical Manager and Medical Director, and support hospice education and performance improvement.

Requirements

• Active RN license in the state where services are provided; work is not permitted in states where the nurse is not licensed.
• Additional state licensure, if required, must be obtained within 60 days.
• One year of hospice and home health experience.
• Current CPR certification; candidates hired after September 28, 2026 must obtain BLS certification within 30 days of hire.
• Ability to perform nursing visits, on-call and triage duties, communicate accurately, and work autonomously and collaboratively.
• Ability to regularly lift or move 25 pounds and occasionally lift or move up to 100 pounds, with the mobility required for field nursing work.
