What you will do

• Provide individualized, age-specific nursing care from admission through discharge.
• Assess patient status, identify care needs, validate diagnoses, and document clinical findings.
• Develop, update, and evaluate individualized plans of care with patients, families, and healthcare professionals.
• Coordinate interdisciplinary care, participate in multidisciplinary conferences, and make referrals for continuity of care.
• Provide evidence-based nursing interventions, patient and family education, and discharge planning.
• Conduct joint home visits with the Charge Nurse and document patient progress and responses to care.

Requirements

• Bachelor of Science in Nursing (BSN), or an Associate Degree in Nursing with a commitment to obtain a BSN within three years of starting the clinical nurse role.
• Current Illinois Registered Professional Nurse license required.
• American Heart Association Basic Life Support (BLS) certification required.
• Strong interpersonal, collaboration, analytical, and clinical documentation skills.
• Active driver’s license and auto insurance required.
