Location: 600 N Westmoreland Rd, Lake Forest, Illinois, USA; onsite ambulatory setting with some travel and after-hours on-call support required.
Salary: USD 38.50–61.60 per hour

What you will do
- Coordinate care for high-, moderate-, low-risk, and newly enrolled patients in an ambulatory setting.
- Review patient records, registries, reports, and claims data to identify care coordination needs.
- Assess clinical, psychosocial, and financial barriers and develop individualized care plans.
- Provide triage, patient and family education, post-discharge outreach, referral management, and transitions of care.
- Collaborate with physicians, clinical support staff, multidisciplinary teams, and community resources.
- Monitor care quality and satisfaction, support improvement initiatives, and provide urgent after-hours care coordination.
Requirements
- Current Illinois Registered Nurse license required.
- BSN required, either at hire or within two years of hire.
- Three years of relevant clinical experience required; five years preferred.
- Experience with chronic conditions, evidence-based medicine, care coordination, care management, and psychosocial or behavioral factors affecting health outcomes.
- Willingness to travel is required; some travel may be necessary.
- Experience with ambulatory nursing, care management, care coordination, and process improvement is preferred.
Nice to have
- Case Management certification or willingness to complete certification when eligible.
- Experience with ambulatory operations and ambulatory nursing care.
- Process improvement experience.
