Focus on conducting patient assessments, managing treatments, supporting new inpatient GIP admissions, and participating in shared on-call coverage.
Location: 15295 E. 127 St., Lemont, Illinois, United States; onsite inpatient and community-based care. Monday–Friday, 11:00 a.m.–3:00 p.m., with shared on-call coverage including nights, weekends, and holidays approximately one week every 13 weeks.
Salary: $61.00–$79.67 per hour

What you will do
- Provide hospice and palliative care in inpatient hospital and community settings.
- Conduct health histories, physical examinations, comprehensive assessments, and face-to-face hospice visits.
- Manage symptoms, evaluate care needs, prescribe or order treatments, and interpret laboratory and diagnostic results within clinical privileges.
- Round on new general inpatient hospice admissions and participate in interdisciplinary team meetings.
- Educate patients, families, staff, and students while serving as a clinical consultant and preceptor.
- Contribute to quality improvement, research, clinical committees, and program development.
Requirements
- Master’s degree from an accredited nursing school or Physician Assistant program.
- Current Illinois Registered Nurse and Advanced Practice Registered Nurse licenses, or a current Illinois Physician Assistant license.
- Acute care certification is required for APRNs; inpatient NP experience may be accepted instead. First Assist certification is required for all APRNs.
- Credentialing and privileges through the Medical Staff Office are required.
- National APRN certification approved by the Illinois APRN Board, or applicable Physician Assistant credentials.
- Basic Life Support certification issued by the American Heart Association is required.
