Home Health RN Case Manager

Location: Malden, Massachusetts and surrounding areas; home-based field work required

Salary: $84,300.22–$107,481.20 annually based on full-time equivalent

What you will do

• Assess patients’ physical, psychosocial, educational, and care needs in their homes.
• Develop and update individualized treatment plans with patients, families, physicians, and interdisciplinary care teams.
• Provide nursing interventions, medication reconciliation, symptom management, supportive care, and complex wound or IV care when applicable.
• Manage an assigned caseload, prioritize visits and telephone triage, and respond to urgent patient or caregiver needs.
• Document assessments, orders, OASIS data, communications, and care provided in accordance with regulatory and payer requirements.
• Coordinate case conferences, continuing services, discharge planning, and community resources while supporting patient safety and avoiding hospitalization.

Requirements

• Active RN license in the state where services are provided; additional required licensure must be obtained within 60 days.
• Valid state-issued driver’s license and ability to travel to patients’ homes in the Malden area.
• At least one year of acute medical/surgical nursing experience.
• Basic Life Support certification; certification timing depends on the hire date.
• Ability to communicate, read, and write in English, work independently, manage priorities, and provide care in a home setting.
• Ability to meet physical requirements, including climbing stairs and regularly lifting or moving 25 pounds and occasionally up to 100 pounds.

Nice to have

• Bachelor of Science in Nursing.
• Specialty nursing certification.
• Three years of acute nursing experience in medical/surgical, oncology, hospice, or home health.
• Experience with IV therapy and complex wound care.
