Care Manager (RN)

Location: Remote; applicants must reside in South Carolina. Work schedule: Monday through Friday, 8:00 AM to 5:00 PM.

Salary: $56,200–$101,000 per year.

What you will do

• Assess member needs, access barriers, social determinants of health, and priorities to determine appropriate care interventions.
• Develop and update personalized care plans and service plans in collaboration with members, families, providers, specialists, and community resources.
• Coordinate care and facilitate timely access to healthcare services, referrals, providers, and local resources.
• Conduct telephonic, digital, home, or other site outreach and monitor member status, changes in condition, and progress toward care goals.
• Educate members and caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Must reside in South Carolina.
• Active RN state license or compact state license required.
• Degree from an accredited school of nursing or a bachelor's degree in nursing.
• 2–4 years of related experience.
• Computer proficiency, strong communication and customer service skills, assessment and critical-thinking skills, care management experience, effective time management, and attention to detail.
