Care Manager (RN)

Location: Remote in Michigan with up to 75% local travel to support members in Wayne and Macomb counties; remaining work may be performed from home. Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs, barriers to care, and social determinants of health, then develop personalized care and service plans.
• Coordinate with members, families, caregivers, providers, specialists, and community resources to address unmet needs.
• Conduct telephonic, digital, home, and other site outreach, including local travel, to evaluate needs and support care delivery.
• Monitor member status, changes in condition, and progress toward care plan goals; update plans when necessary.
• Educate members and caregivers about disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve quality and cost-effective care.

Requirements

• Registered Nurse state licensure and/or compact state licensure required.
• Degree from an accredited school of nursing or a bachelor’s degree in nursing.
• 2–4 years of related experience.
• Ability to support members in Wayne and Macomb counties with up to 75% local travel.
• Ability to work Monday–Friday, 8:00 a.m.–5:00 p.m.
