Care Coordinator II (Healthcare)

Location: Fort Smith, Arkansas; candidates must reside in or near Sebastian or Crawford County, Arkansas, or in Arkansas or a border city. Travel throughout the assigned service area is required, including approximately 30% travel and member home visits.

Salary: $17.84–$28.02 per hour

What you will do

• Conduct member outreach by phone and through home visits to discuss care plans, services, resources, questions, and next steps.
• Coordinate care activities with members, caregivers, healthcare providers, community organizations, practitioners, and care managers.
• Support member and provider inquiries and facilitate continuity of care and services.
• Perform service assessments and screenings, identify member needs, and make referrals to care managers, disease managers, and community-based organizations.
• Educate members about available benefits and resources.
• Document and maintain member records in accordance with state, contractual, and regulatory requirements.

Requirements

• High school diploma or GED and 1–2 years of related experience.
• For the Arkansas Total Care plan: a bachelor's degree in a social science or health-related field, or a high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients.
• Must reside in Arkansas or a border city and be able to travel throughout the assigned service area.
• Valid driver's license, child and adult maltreatment checks before hire and recurring, and a drug screen at hire and recurring are required.
• Ability to follow standards of practice, contractual requirements, and regulatory guidelines.
