Care Navigator - Foster Care

Location: Remote from anywhere within the state of Michigan; may include on-site visits as appropriate. Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.

Salary: $22.94–$38.79 per hour

What you will do

• Assess member needs, barriers to care, available resources, and appropriate care outcomes.
• Develop and maintain personalized care plans and service plans for foster care children.
• Coordinate access to healthcare providers, specialists, community resources, and support services.
• Provide telephonic psychosocial, resource, and benefits education to members, caregivers, and care managers.
• Monitor progress, support care plan revisions, and collaborate with healthcare providers when member needs change.
• Document member information and care management activities in compliance with state, federal, and payer requirements.

Requirements

• Bachelor’s degree and 2–4 years of related experience, or equivalent applicable experience.
• Graduation from an accredited school of nursing is required if holding clinical licensure.
• Current state clinical license preferred.
• Applicants must be able to work remotely from within Michigan.
