Health Home Care Manager (Bronx & Manhattan, NY / Field-based)

Location: Jamaica, NY; field-based across New York, with required travel to patients' homes in Queens and Brooklyn and the ability to travel to at least two boroughs

Salary: $24–$26 USD per hour

What you will do

• Outreach to potentially eligible patients and obtain consent for enrollment in the Health Home Care Management Program.
• Conduct eligibility screenings, initial assessments, reassessments, home visits, and ongoing patient contact.
• Develop, implement, and coordinate patient-centered care plans for people with chronic medical, mental health, and substance use conditions.
• Coordinate referrals and services with providers, pharmacies, insurers, hospitals, family caregivers, and community resources.
• Provide direct support, address access barriers, facilitate appointments, and intervene in crises when needed.
• Document services, care plan outcomes, and patient information in accordance with Health Homes standards while maintaining confidentiality.

Requirements

• Associate's degree required.
• At least two years of Care Management experience.
• Ability to travel in the field and meet patients in person, including travel to at least two New York City boroughs: Brooklyn and Queens.
• Knowledge of local community resources and experience with data reporting, documentation, and outcomes.
• Strong communication, assessment, empathy, and crisis-management skills, with the ability to manage a high-volume caseload.
• Experience navigating multiple data management systems, such as Salesforce.
