What you will do
- Serve as the first telephone contact for patients, caregivers, families, referring physicians, and the community.
- Conduct professional nursing assessments in a high-volume call center and prioritize care according to urgency.
- Coordinate appointments, referrals, screenings, and supportive services with multidisciplinary teams.
- Assist non-clinical staff with clinical questions and collaborate with AHN departments and physician offices.
- Identify at-risk patients and support appropriate screening and care coordination.
- Handle assigned inbound and outbound call projects and maintain accurate patient and payer information.
Requirements
- High school diploma or GED.
- At least one year of nursing experience in the specialty.
- Current Pennsylvania RN license or current multistate licensure through the enhanced Nurse Licensure Compact.
- CPR certification.
- Residence within 50 miles of the Pittsburgh or Erie, Pennsylvania region.
- Act 34, Act 33, Act 73, and other applicable background clearances may be required.
Nice to have
- Bachelor's degree in nursing.
- Experience in an emergency room or ICU, adult medicine or surgery, call center, or telehealth setting.
- Certification in the area of expertise.
- Valid Pennsylvania driver's license, if required for the location.
- Older Adults Protective Services Act clearance, depending on location.
