Care Manager (RN)

Location: Remote from anywhere in the United States; an unrestricted Michigan RN license is required. Monday-Friday, 8am-5pm Eastern Time.

Salary: $56,200-$101,000 per year.

What you will do

• Evaluate member needs, barriers to care, and social determinants of health, prioritizing member-identified goals.
• Develop and update personalized care plans and service plans with members, caregivers, providers, specialists, and community resources.
• Coordinate access to appropriate healthcare, services, referrals, and local resources.
• Monitor member status, changes in condition, and progress toward care-plan goals, revising interventions as needed.
• Provide telephonic, digital, home, or site-based outreach and educate members and families about conditions, care options, provider instructions, benefits, and care gaps.
• Document care-management activities and member information in compliance with state, federal, and payer requirements, and provide feedback on quality improvement opportunities.

Requirements

• Degree from an accredited school of nursing or a bachelor's degree in nursing.
• 2-4 years of related experience.
• Registered Nurse state licensure and/or compact-state licensure.
• Unrestricted Michigan RN license required.
• Applicants must work remotely from within the United States.
