Care Navigator (LPN/LVN) (Healthcare)

Location: Remote from within Missouri, United States; Monday–Friday, 8am–5pm. Occasional on-site visits may be required.

Salary: $22.94–$38.79 per hour.

What you will do

• Assess member needs, barriers to care, available resources, and appropriate care outcomes.
• Develop and maintain personalized care plans and identify providers, specialists, and community resources.
• Coordinate care between members, caregivers, healthcare providers, and care-management teams.
• Provide education and psychosocial support on healthcare benefits, referrals, procedures, and available services.
• Monitor care-plan progress and member status, escalating needs and supporting plan revisions.
• Document member information and care-management activities in compliance with regulatory and payer requirements.

Requirements

• Bachelor’s degree and 2–4 years of related experience, or equivalent applicable experience.
• Graduation from an accredited school of nursing is required when holding clinical licensure.
• Current Missouri LPN/LVN or other state clinical license is strongly preferred.
• Applicants must be able to work remotely from within Missouri.
• Experience supporting children with elevated blood lead levels and their families is relevant to the assignment.
