Care Manager (Clayton, GA / Field-based)

Location: Clayton, Georgia; field-based with frequent travel to patients’ homes and travel within Georgia, including the Atlanta Metro Area

Salary: $22–$26 USD per hour

What you will do

• Conduct home orientations, complete paperwork, and perform initial patient assessments.
• Carry out periodic supervisory visits and check-ins for active cases.
• Train caregivers and patients according to individual case needs.
• Communicate with caregivers, patients, stakeholders, and interdisciplinary care teams.
• Review cases, document observations, and recommend improvements to care procedures.
• Travel throughout Georgia to support field-based operations.

Requirements

• 2+ years of experience working with frail or elderly disabled adults and caregivers.
• Field experience in a medical or case management environment and hands-on home care experience.
• Working knowledge of ADL and IADL assessments.
• Valid driver’s license and an insured vehicle for travel within Georgia.
• Strong verbal and written communication, organization, time management, critical thinking, and problem-solving skills.
• Proficiency in Microsoft Word, Excel, and Outlook; experience with data input, case review, and case note entry.

Nice to have

• LPN licensure.
• Salesforce experience.
• Excellent caregiver training skills and a team-oriented approach.
