Location: Miami, Florida; candidates must reside in the Central or North regions of Miami-Dade County. The role involves visiting members in homes, assisted living facilities, long-term care facilities, and other care settings, with 75–90% local travel.
Salary: $62,000–$66,000 per year based on experience and qualifications. The posted base-pay range is $53,700–$72,600 per year.

What you will do
- Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
- Assess member needs and develop individualized care plans.
- Plan and implement interventions that support health, safety, and well-being.
- Coordinate services with members, families, providers, payer sources, and interdisciplinary teams.
- Connect members with appropriate healthcare, social, and community resources.
- Monitor care plans against members' personal goals and evaluate outcomes.
Requirements
- Bachelor's degree in health services, social work, psychology, or a related field with at least 2 years of relevant experience; or Florida-licensed RN with at least 2 years of relevant experience; or Florida-licensed LPN with at least 4 years of relevant experience; or at least 6 years of health services or case management experience as a case worker.
- Prior experience with Medicare and Medicaid recipients.
- Intermediate to advanced computer skills and experience with Microsoft Office.
- Valid state driver's license and proof of personal vehicle liability insurance required.
- AHCA fingerprinting and Florida Level 2 background screening required.
- Patient-facing role requiring participation in 's tuberculosis screening program.
Nice to have
- Bilingual English/Spanish skills with the ability to pass a language proficiency exam.
- Nursing home diversion or long-term care case management experience.
- Experience with electronic case-note documentation, geriatric populations, health promotion, coaching, and wellness.
- Knowledge of community health and social service agencies; 701B Certification or medication application experience.
