Care Manager (RN)

Location: Remote in Michigan with up to 75% local travel to support members in Wayne and Macomb counties; remote work from home during the remaining time.

What you will do

• Assess member needs, barriers to care, and social determinants of health.
• Develop and update personalized care plans and service plans.
• Coordinate with members, families, caregivers, providers, specialists, and community resources.
• Conduct telephonic, digital, home, and other site outreach, including local travel.
• Provide education on diseases, care options, referrals, healthcare benefits, and care gaps.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2–4 years of related experience.
• RN state licensure and/or compact state licensure required.
• Ability to support members in Wayne and Macomb counties through up to 75% local travel.
• Availability Monday through Friday, 8:00 a.m. to 5:00 p.m.
