What you will do

• Conduct virtual adult and pediatric wellness visits and health assessments.
• Perform health risk assessments, medication reviews, cognitive screenings, and patient education.
• Collaborate with in-home clinical staff to evaluate vital signs, arrange blood draws, and address patient needs.
• Coordinate preventive care, medication management, specialist referrals, and community resources.
• Document clinical findings and maintain high-quality patient care standards.

Requirements

• Board certification as a Family Nurse Practitioner.
• Active Family Nurse Practitioner license in California.
• Credentialed Medicaid provider for California Medi-Cal.
• Prior experience performing wellness visits and the ability to work independently or collaboratively.
• Strong written and verbal communication skills, professionalism, integrity, confidentiality, and compassion.
• Comfort using laptops, mobile devices, and HIPAA-secure messaging tools; successful completion of a national background check and clinical license search.
