RN, Field Care Manager (Physical Health)

Location: Remote home office in Louisiana Regions 1, 2, 3, 4, or 8; primarily field-based with up to 50% travel within the assigned Louisiana region. Work is scheduled Monday through Friday, 8:00 AM to 5:00 PM Central Time.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct telephonic and in-person assessments of members’ physical, environmental, and psychosocial needs.
• Develop and implement personalized care plans based on clinical judgment, health status, and identified barriers.
• Monitor member progress and adapt interventions to support individualized health goals.
• Coordinate clinical services, community resources, social support, and value-added benefits.
• Collaborate with providers, community organizations, primary care teams, and interdisciplinary care professionals.
• Document care activities and incident reports in accordance with organizational and regulatory requirements.

Requirements

• Residence in Louisiana Region 1, 2, 3, 4, or 8 is required.
• Active, unrestricted Louisiana Registered Nurse license is required.
• At least two years of clinical experience with youth, adults, and geriatric populations.
• Knowledge of community health, social service agencies, and community resources.
• Experience with electronic medical records, information applications, narrative documentation, and web navigation.
• Intermediate to advanced computer skills, including Microsoft Word, Outlook, and Excel, with the ability to work independently and exercise sound judgment.

Nice to have

• Bachelor’s degree in nursing and Certified Case Manager certification.
• Experience in health promotion, coaching, wellness, care management, or managed care health plans.
• Bilingual or multilingual ability in English, Spanish, or Creole.
