Manager, Care Management (RN or SW) (Healthcare)

Location: Remote-field position for candidates residing in Michigan, with up to 50% travel to Wayne and Macomb Counties. Core hours are Monday–Friday, 8:00am–5:00pm Eastern Standard Time, with occasional weekend hours.

Salary: $86,300–$118,700 per year, plus eligibility for a bonus incentive plan.

What you will do

• Lead nurses and behavioral health professionals responsible for member care management.
• Oversee member assessments, care coordination, case management, disease management, discharge planning, and post-discharge calls.
• Monitor staff performance and adherence to care coordination benchmarks.
• Develop audit plans, compliance tools, and clinical performance reporting in collaboration with leadership.
• Interview, hire, coach, evaluate, mentor, and onboard care coordination staff.
• Collaborate across departments and travel within Wayne and Macomb Counties to support field clinicians.

Requirements

• Must reside in Michigan and be able to travel to Wayne County or Macomb County.
• Active Michigan Registered Nurse or Social Work license.
• At least 3 years of licensed RN or social work experience, including case or disease management.
• At least 1 year of leadership and people management experience in home care, hospice, or a similar field-based healthcare setting.
• Experience coaching, supporting, and developing clinicians; ability to analyze and interpret data trends.
• Knowledge of Microsoft Word, Excel, and PowerPoint; valid state driver’s license and required personal vehicle liability insurance.

Nice to have

• Associate’s degree or higher.
• Certified Case Manager certification.
• At least 3 years of management or supervisor-level experience.
• Experience with Medicaid Long Term Care.
• Fluency in Spanish or Arabic in addition to English.
