Care Coordinator

Location: Remote in Las Cruces, New Mexico, United States; duties may be performed virtually or face-to-face based on contractual requirements.

Salary: $50,225–$75,335 annual base salary, adjusted according to location, skills, experience, education, and other permitted factors.

What you will do

• Coordinate care for members with behavioral health conditions who require intensive interventions and oversight.
• Conduct comprehensive health risk and needs assessments covering medical, behavioral, psychosocial, environmental, and financial factors.
• Develop, communicate, implement, and regularly monitor individualized care plans.
• Coordinate clinical, community, home care, long-term care, and other support resources while addressing gaps in care.
• Collaborate with members, families, providers, physicians, caregivers, legal representatives, and interdisciplinary care teams.
• Track intervention effectiveness, analyze care trends, generate reports, and support quality and cost-effective outcomes.

Requirements

• 3–5 years of experience in social work, nursing, healthcare, or a related field; relevant experience may substitute for a degree.
• Experience in utilization management, quality assurance, home or facility care, community health, long-term care, or occupational health.
• Knowledge of community and private/public resource referral coordination.
• Ability to analyze data and trends, perform cost/benefit analysis, negotiate, and support cost containment.
• Ability to make decisions requiring significant analysis and original thinking in complex situations.
• High school diploma or GED and a valid in-state driver license required.

Nice to have

• Associate or bachelor's degree.
• Certified Case Manager certification.
• Licensed Clinical Social Worker license.
• Registered Nurse license in a state or compact state.
