Case Manager (Healthcare)

Location: Remote Flex in North Carolina, with a few in-office visits each year; proximity to the Durham headquarters may determine whether the role is Remote Flex or Hybrid Flex.

Salary: $73,698–$117,917 per year, plus annual incentive bonus.

What you will do

• Coordinate care, resources, and services for selected member populations as part of a multidisciplinary team.
• Conduct comprehensive assessments and develop member-centric treatment, intervention, and discharge plans.
• Monitor member progress, arrange follow-ups and referrals, provide education, and modify care goals as needed.
• Coordinate care across physician offices, hospitals, rehabilitation units, skilled nursing facilities, and home care.
• Document assessments, care plans, interventions, monitoring activities, and outcomes.
• Evaluate treatment options, medical necessity, service appropriateness, benefits exceptions, and flex benefit options where applicable.

Requirements

• Registered Nurse with at least 3 years of clinical and/or case management experience.
• Valid and applicable North Carolina or compact multistate clinical licensure is required and must be maintained.
• Clinical skills in care planning, patient assessment, care coordination, healthcare utilization, and patient advocacy.
• Additional specialty certification, such as CCM or CDCES, may be required for some roles and must be obtained within 2 years of employment when applicable.

• Additional non-compact state nursing licenses alongside a compact multistate license.
• Broad nursing experience across medical-surgical, intensive care, emergency, pediatrics, labor and delivery, or other inpatient settings.
