TL;DR
Telephonic Care Coach (Healthcare): Managing members’ health issues by conducting telephonic assessments, identifying care barriers, and coordinating resources with an accent on case management, care planning, and patient education. Focus on monitoring outcomes, resolving barriers to effective care, and supporting Medicare and Medicaid members during CST business hours.
Location: Remote; Illinois residency preferred. Monday–Friday, 8:00am–5:00pm CST, with occasional scheduling flexibility and travel to offices for training or meetings.
Salary: $53,700–$72,600 per year.

What you will do
- Manage members’ health issues using telephonic care coaching strategies and techniques.
- Conduct assessments, identify care needs, and develop active care plans.
- Identify and resolve barriers that prevent effective care.
- Monitor member progress toward desired health outcomes through assessments, data, and conversations.
- Guide members and families to healthcare, community health, and social service resources.
Requirements
- Bachelor’s degree in a health or social service-related field, or LPN licensure with the ability to obtain an Illinois license.
- At least 2 years of case management experience.
- Experience using Microsoft Word, Excel, Outlook, and electronic information systems.
- Illinois residency is preferred.
- Ability to work Monday–Friday, 8:00am–5:00pm CST, with occasional scheduling flexibility.
- Dedicated home workspace required to protect member PHI and HIPAA information.
Nice to have
- Bilingual English/Spanish communication skills.
- Certified Case Manager certification or a master’s degree in a health or social service-related field.
- Experience with Medicare and Medicaid recipients, health promotion, coaching, wellness, discharge planning, or adult acute-care patient education.
- Knowledge of community health, social service agencies, and community resources.
