Clinical Pharmacist, Full-Time and Per Diem Opportunities

Salary: $114,400.00–$141,756.53 annually based on full-time equivalent

What you will do

• Review, interpret, approve, and enter medication orders, checking indications, dosages, allergies, interactions, duplication, and other safety concerns.
• Prepare, compound, label, and dispense medications in accordance with state and federal regulations.
• Provide medication histories, reconciliation, optimization consultations, drug information, and patient and family education.
• Collaborate with physicians, nurses, pharmacists, and multidisciplinary teams on medication-related care and discharge planning.
• Supervise support personnel, maintain controlled-drug processes, and provide operational coverage including evenings, nights, weekends, holidays, and on-call shifts.
• Participate in quality programs, medication-use evaluations, treatment guidelines, clinical pathways, research, and continuing education.

Requirements

• Bachelor’s degree in Pharmacy.
• Active pharmacist license and compliance with applicable state and federal pharmacy regulations.
• At least one year of related experience.
• Experience with electronic medical records and pharmacy computer systems.
• Knowledge of clinical best practices, drug formularies, chronic-condition management, population management, and medication optimization.
• Strong communication, presentation, organization, problem-solving, prioritization, technology, and teamwork skills.

Nice to have

• Doctor of Pharmacy degree.
• Pharmacy residency training.
• Experience conducting home medication visits and inhaler education.
