What you will do

• Identify, prevent, and resolve medication-related problems and coordinate solutions with pharmacists, nurses, prescribers, patients, and other healthcare professionals.
• Monitor drug-drug, drug-diet, and drug-laboratory interactions, therapeutic duplication, adverse reactions, and pharmacokinetic parameters.
• Evaluate medication therapy, review laboratory and microbiology data, adjust treatment when indicated, and document clinical interventions in the electronic health record.
• Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition, while verifying medication orders and maintaining controlled inventories.
• Provide medication education and training to patients, families, healthcare staff, students, residents, and fellows.
• Support formulary management, automated dispensing cabinet restocking, emergency medication management, quality improvement, and medication-use review activities.

Requirements

• Bachelor’s degree in Pharmacy or Doctor of Pharmacy degree.
• Active clinical pharmacist license in Illinois through IDFPR required.
• Knowledge of medication safety, pharmacokinetics, therapeutic drug monitoring, medication compounding, and clinical documentation.
• Ability to communicate professional knowledge effectively in oral and written form.
• Ability to organize workload, collaborate with pharmacists and technicians, and respond to emergency medication-management situations.

Nice to have

• Training or knowledge in specialty areas such as neonatology, critical care, or oncology.
• Completion of a pharmacy residency program or equivalent hospital pharmacy experience.
