Pharmacist Lead, Inpatient Oncology Clinical Support (Oncology Pharmacy)

What you will do

• Act as the pharmacy bridge between BIDMC or Mount Auburn Hospital inpatient teams and Dana-Farber ambulatory oncology care.
• Provide real-time clinical consultation on complex oncology medications and support safe medication practices.
• Coordinate medication and treatment-plan handoffs for high-risk patients transitioning from inpatient to ambulatory care.
• Support inpatient chemotherapy approval, authorization, regimen clarification, and alignment with outpatient treatment plans.
• Collaborate on medication-policy reviews, governance processes, and cross-site quality and safety initiatives.
• Maintain stakeholder relationships, escalation pathways, and communication processes across pharmacy teams.

Requirements

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
• 8 years of hospital pharmacy experience, including at least 6 years in oncology; alternative residency and oncology-experience pathways are accepted.
• Massachusetts pharmacist license required.
• Board Certified Oncology Pharmacist (BCOP) certification required.
• Strong knowledge of clinical oncology pharmacotherapy, medication safety, regulatory standards, communication, and problem-solving.
