Clinical Pharmacist Full Time Rotating

What you will do

• Interpret medication orders and identify allergies, interactions, duplications, incompatibilities, and potential administration errors.
• Monitor medication therapy, pharmacokinetic parameters, laboratory results, cultures, therapeutic drug levels, and adverse reactions; recommend or implement therapy adjustments.
• Compound and dispense sterile products, chemotherapy, parenteral nutrition, and other pharmaceuticals while verifying orders and resolving discrepancies.
• Maintain accurate electronic health records, patient medication profiles, controlled medication inventories, automated dispensing cabinets, and required documentation.
• Provide medication information and education to patients, families, physicians, nurses, students, residents, and other healthcare professionals.
• Participate in emergency medication management, formulary management, quality improvement, medication-use reviews, and centralized and decentralized pharmacy operations.

Requirements

• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy Degree.
• Active clinical pharmacist license in the state of Illinois through IDFPR.
• Experience or proficiency in ER coverage, centralized operations, and decentralized operations.
• Ability to work an 8-hour rotating schedule, including every third weekend and two holidays per year.
• Ability to work cooperatively with pharmacists and technicians and communicate professional knowledge effectively in written and oral form.

Nice to have

• Training or knowledge in neonatology, critical care, oncology, or other specialty areas.
• Completion of a pharmacy residency program or equivalent hospital pharmacy experience.
