Field Care Coordinator- Bilingual Preferred

Location: Must live in Collier County, Florida; remote role with 75–90% local travel to members’ homes, assisted living facilities, long-term care facilities, and other care settings. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $53,700–$72,600 per year

What you will do

• Assess member needs and establish individualized care plans.
• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Coordinate healthcare, social, and community services with providers, payer sources, and interdisciplinary teams.
• Implement interventions and monitor care plans against members’ personal goals.
• Identify and resolve barriers to effective care while supporting optimal wellness.
• Guide members and families toward appropriate care and community resources.

Requirements

• Must live in Collier County, Florida.
• Bachelor’s degree in a health and human services field.
• 2+ years of related experience.
• Exceptional verbal and written communication skills.
• Ability to use electronic information applications and software programs.
• Valid state driver’s license, reliable vehicle, and personal vehicle liability insurance with minimum limits of 100,000/300,000/100,000; the role is part of the driver safety program.

Nice to have

• Experience with electronic case notes and documentation across multiple systems.
• Experience working with Medicare and Medicaid recipients.
• Experience in health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and community resources.
• Bilingual Spanish/English communication skills.
