Care Manager - Behavioral Health

Location: Remote, Oklahoma; must reside in Oklahoma. Periodic travel to the hirify.global Oklahoma City office and travel within the state may be required.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct telephonic and face-to-face assessments of members’ behavioral, physical, environmental, and psychosocial needs.
• Develop care plans and monitor progress toward desired wellness outcomes.
• Guide members and families toward appropriate healthcare, social, and community resources.
• Identify and resolve barriers to effective care through ongoing assessment and evaluation.
• Collaborate with providers, community services, community health workers, housing support specialists, and other care management staff.
• Use clinical judgment to determine when face-to-face support and coordinated in-person meetings are needed.

Requirements

• Must reside in Oklahoma.
• Active Registered Nurse license or Licensed Master Level Social Worker license, including LCSW, LPC, or LMFT.
• At least one year of behavioral health clinical experience.
• At least two years of in-home case management or care management experience.
• Experience coordinating services, care needs, or benefits for Medicaid and/or Medicare enrollees.
• Knowledge of community health, social service agencies, community resources, and Microsoft Office applications.

• Case Management Certification (CCM).
