Community Health Worker (Medicaid)

Location: Must reside in Illinois. Hybrid role with up to 75% travel to members' homes, provider organizations, and hirify.global offices in Chicago and Schaumburg. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $46,100–$62,300 per year

What you will do

• Conduct in-person assessments of members' care needs, preferences, socioeconomic barriers, and home environments.
• Help members schedule physical and behavioral health appointments and address barriers to attendance.
• Navigate healthcare and social-service systems and coordinate access to housing, food, income, transportation, and other basic needs.
• Advocate for members with providers, schools, community resources, and other organizations.
• Provide culturally appropriate health education, health coaching, motivational interviewing, and support for care-plan adherence.
• Conduct outreach, support health-risk screenings and welcome calls, and contribute to readmission-prevention and emergency-department-diversion programs.

Requirements

• Must reside in Illinois and be able to travel extensively, including up to 75% travel.
• At least two years of experience working with community resources, community health agencies, or social-service agencies.
• Valid driver's license, personal automobile, and vehicle insurance meeting state requirements or hirify.global's specified minimum limits.
• Ability to work 40 hours per week, Monday through Friday, from 8:00 AM to 5:00 PM; overtime may be requested.
• Ability to multitask in a fast-paced environment and demonstrate respect for diverse cultures and demographics.
• Dedicated home workspace and internet service with at least 25 Mbps download and 10 Mbps upload speeds to protect member PHI and HIPAA information.

Nice to have

• Bachelor's degree in social work or a related health field.
• Community Health Worker training or certification.
• Familiarity with state Medicaid program guidelines.
• Experience working with Medicaid enrollees and members with physical or behavioral health needs.
