LTSS Service Care Manager - Behavioral Health

Location: Hybrid role based in Texas, serving Pearland and surrounding counties; includes home and site visits with up to 75% monthly travel. Work schedule is Monday–Friday, 8:00 a.m.–5:00 p.m. CST, with no on-call, evening, weekend, or holiday work.

Pay: $27.02–$48.55 per hour

What you will do

• Evaluate complex and high-risk members with mental and behavioral health needs and develop or support individualized care plans.
• Act as a liaison and advocate for members and coordinate with families, physicians, facilities, agencies, and community providers.
• Conduct frequent home and site visits to assess needs, collaborate with resources, and support member engagement.
• Educate members, families, and caregivers on preventive care, treatment options, medications, referrals, benefits, and available services.
• Coordinate medical, behavioral, and social resources, including housing, transportation, meals, employment, foster care, and activities of daily living.
• Document care management activities, support compliance with clinical and regulatory guidelines, and identify opportunities to improve quality and cost-effectiveness.

Requirements

• Master’s degree in Mental Health Counseling or Social Work required.
• Unrestricted behavioral health professional license required in the applicable state, such as LCSW, LMFT, LMHC, or LPC.
• Two to four years of related experience required.
• Strong clinical documentation, communication, problem-solving, organization, and time-management skills.
• Ability to work independently with minimal supervision and adapt to changing business needs.
• Proficiency with web-based applications and case management systems, including Microsoft Office tools.

Nice to have

• Four or more years of case management experience, especially in child welfare, foster care, adoption, pediatric acute care, schools, behavioral health agencies, or inpatient treatment.
• Experience supporting Foster Care Medicaid recipients and collaborating with medical and behavioral health providers.
• Field-based experience, high-volume caseload management, or managed care experience.
• Bilingual English/Spanish skills.
