Registered Nurse, Part-Time (New Haven, CT / Field-based)

Location: New Haven, Connecticut; field-based with frequent travel to patients' homes across the assigned area.

Salary: $90,000–$100,000 per year; compensation range also listed as $45–$50 USD.

What you will do

• Conduct initial in-person clinical assessments, patient and caregiver orientations, and service activation.
• Develop, implement, manage, and revise individualized patient care plans.
• Coordinate care with physicians, interdisciplinary professionals, community resources, patients, and caregivers.
• Provide caregiver coaching, education, and support while reviewing caregiver notes and responding to changes in patient condition or environment.
• Monitor care quality, patient and caregiver well-being, safety, health outcomes, and regulatory compliance through scheduled home visits.
• Document clinical information and communicate concerns, changes in condition, and barriers to care for timely intervention.

Requirements

• Diploma from an accredited nursing program.
• Valid and active Registered Nurse license in the State of Connecticut.
• Ability to travel by car within the assigned geographic area; a valid driver's license and car insurance are required.
• Ability to prepare effective case summaries and communicate clinical information clearly.
• Proficiency with common software and web-based applications, including case management systems and Microsoft Office.
• Strong customer service skills, empathy, active listening, and patience.
