Registered Nurse, Part-Time (West Hartford, CT / Field-based) (Home Care)

Location: West Hartford, Connecticut, United States; field-based with frequent travel to patients' homes

Salary: $90,000–$100,000 per year; compensation range of $45–$50 USD

What you will do

• Conduct initial in-person clinical assessments and coordinate service activation.
• Develop, implement, manage, and revise individualized patient care plans.
• Coordinate care with physicians, other disciplines, community resources, patients, and caregivers.
• Provide caregiver coaching, education, and connections to appropriate resources.
• Review caregiver notes and address changes in the patient's condition or environment.
• Monitor care quality and maintain compliance with regulatory standards and company procedures.

Requirements

• Diploma from an accredited nursing program.
• Valid and active Registered Nurse license in Connecticut required.
• Ability to travel by car within the assigned geographic area; valid driver's license and car insurance required.
• Ability to prepare effective case summaries and communicate clinical information clearly.
• Proficiency with case management systems, Microsoft Office, and web-based applications.
• Empathy, active listening, patience, and a strong customer-service orientation.
