Registered Nurse Hospice Inpatient Part Time Days

Location: Chicago, Illinois, United States; onsite patient-care role with joint home visits

Salary: USD 34.50–55.20 per hour

What you will do

• Provide individualized, age-specific nursing care from admission through discharge.
• Assess patient status, identify care needs, validate diagnoses, and document clinical information.
• Develop, implement, and continuously update patient-centered plans of care.
• Coordinate interdisciplinary care, referrals, conferences, and continuity-of-care activities.
• Provide evidence-based patient and family education and evaluate progress toward outcomes.
• Perform joint home visits with the Charge Nurse and communicate care updates to patients, families, and healthcare professionals.

Requirements

• Bachelor of Science in Nursing required; an Associate Degree in Nursing is acceptable with a commitment to obtain a BSN within three years of starting the clinical nurse role.
• Current Illinois Registered Professional Nurse license required.
• American Heart Association Basic Life Support certification required.
• Strong interpersonal, collaboration, and analytical skills.
• Active driver’s license and auto insurance required.
