Care Coordinator III (Medicaid)

Location: Fully remote within specified southern Illinois counties, with 10% travel to a local office for quarterly team meetings. Work schedule: Monday-Friday, 8am-5pm Central Time.

Salary: $20.39-$34.71 per hour.

What you will do

• Coordinate care activities for Illinois Medicaid YouthCare members according to individual care and service plans.
• Conduct member outreach by phone and through home visits, provide education, and discuss resources, next steps, and care concerns.
• Work with care managers, healthcare practitioners, community organizations, providers, partners, members, and caregivers to support continuity of care.
• Perform service assessments and screenings for members with complex needs, document care needs, and maintain member records.
• Respond to member and provider inquiries, triage and escalate complex requests, and make referrals to care managers, community organizations, and disease managers.
• Follow contractual, regulatory, and organizational standards and contribute to training and development activities when needed.

Requirements

• Residence in one of the specified southern Illinois counties is required.
• Bachelor’s degree in nursing, social sciences, social work, or a related human-services or social-services field for the Illinois YouthCare plan.
• One year of supervised clinical experience in a human-services field.
• High school diploma or GED and 2-4 years of related experience.
• Ability to work Monday-Friday from 8am to 5pm Central Time and attend quarterly meetings at a local office.
