Care Coordinator III (Healthcare)

Location: Fully remote, but applicants must reside in Illinois; 10% travel to a local office for quarterly team meetings. Work schedule: Monday-Friday, 8am-5pm Central Time.

Salary: $20.39-$34.71 per hour.

What you will do

• Conduct member outreach by phone and home visits to discuss care plans, services, resources, education, and next steps.
• Coordinate care activities with care managers, healthcare providers, community organizations, members, and caregivers.
• Perform service assessments and screenings for members with complex needs and document care requirements.
• Respond to member and provider inquiries, triage complex requests, and escalate issues to management.
• Maintain member records in accordance with state, contractual, and regulatory requirements.
• Identify needs and make referrals to care managers, community organizations, and disease managers.

Requirements

• Must reside in Illinois, specifically one of the listed eligible counties.
• Bachelor's degree in nursing, social sciences, social work, or a related human-services field for the Illinois YouthCare plan.
• One year of supervised clinical experience in a human-services field.
• High school diploma or GED and 2-4 years of related experience.
• Ability to work Monday-Friday, 8am-5pm Central Time.
• Ability to perform member outreach, care coordination, documentation, referrals, and support for complex cases.
