Behavioral Health Case Management Coordinator (Behavioral Health)

Location: Remote within Illinois; occasional travel of up to 2% for onsite presentations at the Downers Grove office. Work hours are within 8:00 AM–5:00 PM CST, Monday through Friday.

Base pay: $61,500–$136,100 annually, plus eligibility for an annual incentive bonus.

What you will do

• Perform behavioral health case management according to established department criteria.
• Assess, plan, coordinate, monitor, and evaluate services for individuals and families.
• Consult with physicians, providers, members, and other resources to support quality and cost-effective outcomes.
• Advocate for appropriate care options and serve as a liaison between members, providers, and physicians.
• Represent the dedicated custom team during client audits and presentations.
• Collaborate with interdisciplinary teams and adapt to changing requirements.

Requirements

• Registered Nurse, or a master’s-level behavioral health professional or doctoral-level behavioral health professional with an independently practiceable license.
• Current, valid, unrestricted clinical license to practice in Illinois is required.
• At least two years of post-licensure clinical experience at the independent practice level providing direct clinical care.
• Experience in utilization management, discharge planning, or managed care.
• Customer service, verbal and written communication, PC, and database skills.
• Visa sponsorship is not available.

Nice to have

• Case Management Certification (CCM).
• Three years of clinical practice experience.
• Experience with ancillary services, provider-side insurance, utilization management, case management activities, or standardized criteria sets.
• Bilingual English/Spanish or English/Polish communication skills.
• Client-facing experience, presentation skills, problem-solving ability, and proficiency with Microsoft Teams and virtual training tools.
